Which MSE Fields Are Single-Select vs Multi-Select?
In real documentation, some observations can coexist while others usually need one primary descriptor. This tool uses multi-select for fields where multiple observations may be true, and single-select for fields that are usually summarized as a primary clinical description.

Mental Status Exam Descriptors
MSE descriptors should be observable, specific, and edited for the actual encounter. Use neutral documentation language and avoid turning template phrases into a diagnosis.
How to Use the MSE Template Safely
- Edit every generated note. The output is a draft, not a finished clinical record.
- Document observations. Separate what was observed, what was reported, and what requires further assessment.
- Follow local policy. Risk assessment language, required fields, and escalation steps vary by organization and jurisdiction.
- Do not infer risk automatically. The tool only formats the risk items you select; it does not decide safety level from MSE descriptors.
- Protect privacy. Avoid entering names, identifiers, or sensitive details into tools unless your workflow permits it.
MSE Template vs Mini Mental Status Exam
A mental status exam template is a broad clinical documentation framework. The Mini-Mental State Examination, often searched as mini mental status exam, is a specific scored cognitive screening instrument. This page does not reproduce or score the MMSE.
